About the Service

This service enables customers to obtain a replacement of a lost original death certificate.

Service Process

  • 1
    Apply for the service through MOHAP Website, MOHAP Smart Application or by visiting the public health center and paying the fees.

  • 2
    Public health center verifying the documents and original copies, and issuing the death certificate.

  • 3
    Receiving a physical copy of the certificate by a Direct relative of the deceased person through public health centers (an e-copy can be delivered through email upon customer preference).

Conditions & Requirements

  1. A relative of the deceased must be present to obtain the certificate.

  2. Copy of the death certificate, if any.

  3. ID card of the certificate recipient.

  4. Passport, ID or Family book( for Nationals).

  5. In the case of companies, a representative of the company must be present.

  6. Request must be made at the same preventive medical center from which the original death certificate was issued.

FAQ’s

  1. Where can I issue a replacement for a death certificate?
    It can be issued from the same public health center from which the original certificate was issued or through the Ministry’s website.
  2. Should the recipient of the death certificate be from the direct relatives of the deceased person?
    Yes, if there are direct relatives of the deceased person at UAE.
    In the absence of direct relatives of the deceased person, the following people can receive the certificate:

    • A person with legal power of attorney from the relatives of the deceased, indicating that the person holding the attorney can receive the certificate.

    • Sponsor if the deceased person holds a valid residence VISA from UAE.

    • A staff member of the Consulate of the Country of the deceased person, under a condition of carrying an official letter from the Consulate, stating that the member can receive certificate.
